2013 Fondy JFL Registration

It is time for the 2013 JFL Registration. What you will need to do is fill out the following:

- Player Registration Sheet

- Volunteer Sheet

- Coaches Questionaire
(If you are interested in helping coach a team)

- Code of Conduct form
(Player and Parent must read and sign)

Also, please print and read the "Family Participation Letter"

The Player Registration Sheet and SAMPLE Medical Release form is for VIEWING ONLY. You will need to fill out a real 2 sided forms at registration.
